This case involves a patient that has a sleep study that shows both respiratory events and periodic limb movements. She also has no alpha rhythm. The definition of sleep onset in patients without alpha rhythm is reviewed.

Discussion of an article by Gupta and Simpson on the relationship between OSA and psychiatric disorders. The discussion includes exploration of cause and effect and a model of “feed forward” reactions that appear to worsen symptoms of both disorders.
